New Patient Registration

If you would like to register with our practice you must live within the practice catchment area.

You can collect a New Patient Pack from reception or you can print from the links below. Please note that the links below should be printed and completed prior to handing into the Surgery. 

You are required to complete three forms below: 1. registration form - 2.new patient questionnaire form - 3.new patient additional information form.

Once you have completed your forms, you should bring these in person to reception for processing. You will be asked to provide proof of your UK residency and address, please bring photographic identity (eg passport) and a utility bill showing your current address.

Following registration it may take some time for your medical records to arrive from your previous GP.  To ensure continuity of care, you may be offered an appointment with our healthcare assistant who will take details of your past medical history, any medication you are taking and carry out a simple health check.

Please note that you will be registered with the practice and not an individual GP, although patients are welcome to consult with the doctor of their choice where appropriate.
