New Patient Registration

If you would like to register with our practice you must live within the practice catchment area.

You will be asked to complete a registration form and new patient assessment form.  When you hand in your completed form to us you will be asked to provide proof of your UK residency and address, please bring photographic identity (eg passport) and a utility bill showing your current address.

Following registration it may take some time for your medical records to arrive from your previous GP.  To ensure continuity of care, you will be offered an appointment with our healthcare assistant who will take details of your past medical history, any medication you are taking and carry out a simple health check.

Please note that you will be registered with the practice and not an individual GP, although patients are welcome to consult with the doctor of their choice where appropriate.

Change of Personal Details

If you change your name, address or telephone number, please notify us immediately so that we can amend your records.
